Admission Process

Pinecrest HavenAdmission Process

We make transitioning to Pinecrest Haven smooth and stress-free.

STEPS:

  • Step 1: Inquiry & Initial Contact Contact us by phone or email to discuss your needs and get information.
  • Step 2: Tour & Consultation Schedule a guided home tour and meet our team.
  • Step 3: Assessment & Review We complete an intake assessment to understand care needs, preferences, and support level.
  • Step 4: Documentation Submission, Required forms include: • Admission Application • Health & Medication Records • PCP/Case Manager referral (if applicable) • Guardianship/POA documentation
  • Step 5: Final Review & Move-In Once approved, we finalize your service plan and welcome you home.

  • Clothing & personal care supplies
  • Favorite bedding or comfort items
  • Identification & insurance information
  • Medication bottles & MAR
  • Contact list for family/guardians
