- Step 1: Inquiry & Initial Contact Contact us by phone or email to discuss your needs and get information.
- Step 2: Tour & Consultation Schedule a guided home tour and meet our team.
- Step 3: Assessment & Review We complete an intake assessment to understand care needs, preferences, and support level.
- Step 4: Documentation Submission, Required forms include: • Admission Application • Health & Medication Records • PCP/Case Manager referral (if applicable) • Guardianship/POA documentation
- Step 5: Final Review & Move-In Once approved, we finalize your service plan and welcome you home.
